Our focus is enabling providers to improve care delivery, one patient at time. Hospitalizations are difficult events that can suddenly interrupt life. LHP takes a holistic approach to patient care with an emphasis on bringing together providers throughout the inpatient and post discharge care continuum. We are helping patients and their physicians plan beyond the hospital door so that the healing and recovery process continues after discharge.
