Summary of LHP Services

LHP is focused on developing and providing the support services necessary for front line providers to effectively manage the patients’ hospital care. We deliver post-acute care more efficiently with innovation, infrastructure and improved patient outcome delivering savings in a value-based payment environment.  These services include:

• Data analytics and reporting to identify trends, benchmark, and report results;
• IT infrastructure connecting our partner providers with post-acute providers (e.g., LTACs, SNFs and HHAs) and other providers integral to the patient care process;
• Discharge planning and coordination teams to facilitate appropriate transitions from acute to post-acute care;
• Condition specific patient management protocols and negotiations with ancillary providers involved in care to enhance overall program results;
• Care management processes, systems and personnel dedicated to effectively supporting the patient pathway.
